Psychedelic Therapy for Depression

Major depressive disorder (MDD) and treatment-resistant depression are among the most studied conditions in psychedelic medicine. Psilocybin has received FDA Breakthrough Therapy Designation for treatment-resistant depression, with COMPASS Pathways running Phase 3 trials and ketamine already approved as Spravato. Clinical evidence points to rapid, durable antidepressant effects — often after just one to three sessions.

Research suggests psychedelics work differently than conventional antidepressants: rather than requiring daily dosing to maintain effects, psilocybin and ketamine appear to trigger neuroplasticity and break rigid thought patterns associated with depression, with benefits lasting weeks to months after a single session.

Phase 2

Psilocybin matched a leading antidepressant — with about twice the remission rate

In a phase 2 head-to-head trial, two psilocybin sessions and six weeks of daily escitalopram produced similar average symptom change. Remission was 57% with psilocybin versus 28% with escitalopram — though the difference on the study’s primary measure was not statistically significant.

Phase 2

A single 25 mg dose reduced treatment-resistant depression

In the largest psilocybin depression trial to date, a single 25 mg dose of synthetic psilocybin (COMP360) with psychological support significantly lowered depression scores over three weeks compared with a 1 mg control, in people whose depression had not responded to standard treatment. Adverse effects were reported.

Phase 2

Rapid and sustained antidepressant effect in major depression

In a randomized trial of 104 adults, 25 mg psilocybin given with therapy produced a rapid and sustained reduction in depressive symptoms over six weeks compared with a delayed-treatment control, with no serious treatment-related adverse events.
